Position: Case Management Specialist

               (Licensed Case Manager/ Licensed Mental Health Worker)

Status: Full-time

 

Position Summary

The Community Based Case Management (CBCM) Licensed Case Management Specialist shall be a member the multi-professional team responsible for ensuring all consumers are provided with a complete an integrated consumer-centered intake assessment. The assessment shall include, but not be limited to, consumer strengths, preferences, abilities, needs, and historical data regarding the consumer’s family, social support, cultural influences and medical status including psychiatric illness, substance abuse legal; employment, education, abuse and neglect issues, crisis planning and activities of daily living. The CBCM Licensed Case Management Specialist shall be responsible for coordinating the development of and monitoring the implementation of the Individualized Recovery Plan (IRP) and shall act as the communication liaison for the Case Management (CM) team both internally and externally with respect to the IRP. The CBCM CM to consumer ratio shall not exceed one 1:40, the ratio assumes a caseload mix of consumers with varying needs and that the majority of the consumer population of the caseload is relatively stable and in active stages of recovery; Under the direct supervision of the CBCM Team Leader/Psychiatrist or APRN Rx

 

Minimum Education                                      

Possesses one of the following:  Current Hawaii licensed social worker (LSW), licensed clinical social worker (LCSW), licensed marriage and family therapist (LMFT), licensed mental health counselor (LMHC), licensed advanced practice registered nurse (APRN), Hawaii licensed registered nurse with two years of clinical experience in the behavioral health field, current Hawaii licensed practical nurse (LPN) with two years of clinical practice experience in the behavioral health field or current Hawaii licensed registered nurses with two years of clinical practice experience in the behavioral health field.

 

Minimum Field-of-Expertise                         

Strong clinical skills and experience demonstrating competence in problem-solving, critical thinking, collaboration, and clinical competencies in providing education and training to staff members.  Computer proficiency in various software.

Position: Case Management Specialist

Status: Full-time

 

Position Summary

Provide specialized case management services for clients diagnosed with serious and persistent mental illness.  Responsibilities include, but are not limited to, psychosocial assessment, needs assessment, creating recovery plans, providing education and support for the client and others in the client’s significant support network, linking clients to community resources to further independence and stability and working collaboratively with fellow team members. 

 
Minimum Education

Must have a minimum of a Bachelor’s degree with a minimum of twelve (12) semester credit hours in courses such as counseling, criminal justice, human services, psychology, social work, social welfare, sociology, or other behavioral sciences.  Must have one and one-half (1½) years of specialized experience working with persons with severe and persistent mental illnesses.

 

Minimum Field-of-Expertise

Must meet minimum educational and/or experience as noted above. Clinical skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program clients with severe and persistent mental illnesses under the clinical supervision of the Team Leader.  Need ability to independently respond to the varied needs and demands of clients, families/significant others, staff, physicians and other multidisciplinary team members.  Must have strong commitment to the rights and abilities of each client served.

Position: Registered Nurse (Case Management)

Status: Full-time

 

Position Summary
The Community Bases Case Management (CBCM) RN shall be a member the multi-professional team responsible for conducting psychiatric and medical assessments, basic health care, education, and coordination of medical needs; coordination of appointments/referrals with community physicians, manage, and monitor psychotropic medication administration under the supervision/order of the psychiatrist. The RN shall have an active role in the provision of case management and rehabilitation Services. The RN shall work with the consumer and their support system on the management of symptoms, the development of wellness responses to co-morbid conditions, and development of CM team skills to manage complex health conditions, under the direct supervision on the Care Hawaii, CBCM Team Leader and/or CBCM Psychiatrist.  The RN to consumer ratio shall not exceed one (1) FTE position to one-hundred fifty consumers

 

Minimum Education

Must be a Licensed Registered Nurse in the State of Hawaii, Master’s degree preferred, may have Bachelors or Associate degree, and have two (2) years experience, preferably in mental health

 

Minimum Field-of-Expertise

Strong clinical skills and experience demonstrating competence in problem-solving, critical thinking, collaboration, and clinical competencies in providing education and training to staff members

Program: LCRS

Position: Residential Crisis Manager

Status: Call-in

 

Position Summary

This member of residential services team has a responsibility to carry out rehabilitation and support functions, and to assist in treatment and crisis intervention; also provides substance abuse services, education, support, and consultation to residents and families as needed.  The residential crisis manager facilitates the admission and discharge process for the assigned site, in collaboration with the Registered Nurse on duty, case management and/or crisis worker
 

Minimum Education

Must have a Bachelor’s degree in one of the behavioral health sciences or a Bachelor’s degree in unrelated field with 12 or more credit hours in behavioral health sciences
 

Minimum Field-of-Expertise

1 ½ years of specialized work experience required

Program: LCRS

Position: Registered Nurse (Residential)

Status: Call-in

 

Position Summary

This member of the multidisciplinary team is responsible for conducting psychiatric assessments; assessing physical health needs; making and supporting appropriate referrals to community physicians and for mental health services; providing management and administration of medications in conjunction with the psychiatrist; providing a range of contracted treatment, rehabilitation, and support services; and sharing shift-management responsibility with other staff.
 

The Residential RN shall be supervised and will work collaboratively under the direction of the Medical Director, Director of Crisis, Nursing Director & Team Leader to consult and resolve the consumers’ emergent nursing and psychiatric needs consistent with nursing and mental health best practices

 

Minimum Education

Must meet the following minimum educational requirements: Associate’s degree in Nursing

 

Minimum Field-of-Expertise

Four (4) years of nursing experience, One (1) year of Psychiatric experience in an inpatient or outpatient psychiatric setting.
