Position: Counselor

Status: Full-time

 

Position Summary

The Counselor position is responsible for a variety of prevention/treatment services to include: referral, screening, assessment, intake and orientation, individual, family and discharge planning, crisis management, counseling (individual and group), education, case management, charting, and community outreach.  Additionally, the Counselor has knowledge in theater, music, creative writing, arts and crafts.

 

Minimum Education

High School Diploma or GED. and 3 years of professional, counseling experience; or Bachelors Degree in Psychology, Sociology, Social Work or a Human Service-Related Field and 1 year of professional counseling experience, and Certified Substance Abuse Counselor (CSAC) is preferred through the State Department of Health, Alcohol and Drug Abuse Division.

 

Minimum Field-of-Expertise

At least 3 years professional experience working with clients who have substance abuse problems,   with a High School Diploma or GED or at least 1 year professional experience working with clients who have substance abuse problems, with a Bachelor’s Degree.

Program: CBCM

Position: RN (Case Management)

Status:  Full-time

 

Position Summary

The Community Bases Case Management (CBCM) RN shall be a member the multi-professional team responsible for conducting psychiatric and medical assessments, basic health care, education, and coordination of medical needs; coordination of appointments/referrals with community physicians, manage, and monitor psychotropic medication administration under the supervision/order of the psychiatrist. The RN shall have an active role in the provision of case management and rehabilitation Services. The RN shall work with the consumer and their support system on the management of symptoms, the development of wellness responses to co-morbid conditions, and development of CM team skills to manage complex health conditions, under the direct supervision on the Care Hawaii, CBCM Team Leader and/or CBCM Psychiatrist.  The RN to consumer ratio shall not exceed one (1) FTE position to one-hundred fifty consumers.

 

Minimum Education

Must be a Licensed Registered Nurse in the State of Hawaii, Master’s, Bachelors or Associate degree, and have two (2) years experience, in mental health nursing.

Program: CMO

Position: Crisis Therapist

Status:  Full-time & Part-time

 

Position Summary
This member of the Crisis Services team has advanced training in one of the mental health professions (e.g., nursing, social work, psychology) and clinical skill and experience to provide crisis stabilization and crisis support management to clients experiencing immediate and often life-threatening crisis situations.  Work is primarily intellectual in nature and requires the consistent exercise of discretion and judgment; requires ability to function independently in the assessment of suicidality/homicidality or other risk assessment

 

Minimum Education

Must meet the following minimum educational requirements:  Bachelor’s Degree with a minimum of 12 semester credit hours in counseling, criminal justice, human services, psychology, social work, social welfare, sociology, or other behavioral sciences and 1 ½ years of specialized experience

 

Minimum Field-of-Expertise

Post-secondary school education and training is required, including internships and other supervised work/practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illnesses; advanced knowledge in psychology, risk management and behavior management required

Position: Crisis Therapist

Status:  Full-time

 

Position Summary

Utilize clinical skills and experience to provide time-limited crisis support to a caseload of clients identified as experiencing immediate and often life-threatening crisis situations. Provide community based services.  Participate in the Crisis Mobile Outreach on-call schedule as needed.  Availability for weekend and night shifts.

 

Minimum Education

Requires at least Bachelor’s degree (with a minimum of 12 semester credit hours in counseling, criminal justice, human services, psychology, social work, social welfare, sociology, or other behavioral sciences) and 1½ years of specialized experience

 

Post-secondary school education and training is required, including internships and/or other supervised practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illnesses.  Advanced knowledge in psychology, risk management, and behavior management required.  Must be computer proficient.

Program: LCRS

Position: Registered Nurse (Residential)

Status: Call-in

 

Position Summary

This member of the multidisciplinary team is responsible for conducting psychiatric assessments; assessing physical health needs; making and supporting appropriate referrals to community physicians and for mental health services; providing management and administration of medications in conjunction with the psychiatrist; providing a range of contracted treatment, rehabilitation, and support services; and sharing shift-management responsibility with other staff.
 

The Residential RN shall be supervised and will work collaboratively under the direction of the Medical Director, Director of Crisis,  Nursing Director & Team Leader to consult and resolve the consumers’ emergent nursing and psychiatric needs consistent with nursing and mental health best practices

 

Minimum Education

Must meet the following minimum educational requirements: Associate’s degree in Nursing

 

Minimum Field-of-Expertise

Four (4) years of Nursing experience, One (1) year of Psychiatric experience in an inpatient or outpatient psychiatric setting.

Position: Mental Health Worker

Status: Full-time & Call-in

 

Position Summary

The mental health worker for SRSP will assist clients and their families identify their problems, cope with their conditions and function effectively within their environment.
 

Minimum Education

Must have a Bachelor’s degree with a minimum of 12 semester credit hours in courses such as counseling, criminal justice, human services, psychology, social work, social welfare, sociology or other behavioral sciences

 

Minimum Experience

1 ½ years of specialized experience working with individuals with mental illness required.  Specialized experience is progressively responsible professional work experience that involved helping individuals and their families find satisfactory ways of indentifying their problems, coping with their conditions and functioning effectively within their environment.  This experience may include identification and evaluation of the client’s problems and needs, the development of a service or treatment plan, the initiation and implementation of the treatment plan, monitoring of services and evaluation/assessment of the client’s progress.  Also a mental health worker will either be cross-trained in the provision of treatment to individuals with co-occurring mental and substance abuse disorders and/or be certified as a substance abuse counselor (CSAC).

Position: Mental Health Technician

Position Summary

This member of the multidisciplinary team has a responsibility to carry out rehabilitation and support functions such as psychosocial rehabilitation programming, treatment planning, education, support, consultation to families, and provide crisis intervention under the clinical supervision of staff with professional degrees.  Additional duties may include collaboration with a multidisciplinary treatment team, processing utilization, admission and discharge paperwork and preparing reports.
 

Minimum Education

Have a minimum of a high school diploma or high school equivalency and completion of twelve (12) credits or four (4) classes of college coursework in social work, human services, human development, psychology, or a closely related field
 

Minimum Field-of-Expertise

Have one (1) year experience in the provision of psychological, social, or counseling services in a human service delivery setting that includes interviewing consumers and/or observing and recording behaviors for use in the development and implementation of treatment, rehabilitation or service plans.

Position:  Behavioral Specialist

Position Summary

The primary duties of the Behavioral Specialist are working with persons with disabilities in assessment, individual planning, and training regarding behaviors. The duties shall include conducting Functional Behavioral Assessments (FBAs) and/or the development of a Positive Behavioral Support (PBS) plans. The Behavioral Specialist may act as the clinical supervisor if working on an intensive case in which PAB III’s are assigned to a participant.  This member may work in collaboration with DOH Case Managers, a Clinical Supervisor, a Service Supervisor, a Dietician, and any other members of the participant’s circle of support

 

Minimum Education

Master’s Degree in the behavioral sciences, education, nursing, or therapeutic field from an accredited university or college with at least two (2) year of experience
 

Minimum Field-of-Expertise

This includes but not limited to support, training and consultation to families, and provides behavioral interventions under the clinical supervisor, registered nurse and/or resident manager’s direction.  Additional duties include collaboration with a multidisciplinary treatment team to modify maladaptive behaviors of the resident, processing utilization, admission and discharge paperwork and preparing reports

Position:  PAB I and II

Position Summary
The Personal Habilitation Worker’s (PAB) purpose is to facilitate the participant’s involvement in the home and community setting and maximize their independence.  Participates in regular supervision and training activities provided by the Developmental Disabilities Program.  The PAB must have the ability to deliver services in various environments, such as homes, schools, community, street locations, etc.  This position involves hands on assistance (actually performing the task with or for the person) or training or multi-step instructional cuing as a part of a plan to prompt the Participant to perform a task.  Services include personal assistance, training to meet outcomes/goals agreed upon by the participant, Case Manager and PAB such as increasing independence, developing natural supports, learning daily living skills and developing relationships

Location:  Oahu

Position:  Service Supervisor

Status: Full-time

 

Position Summary
This member of the DD Medicaid Waiver/CNS program of the C&A Department has a responsibility for creating the participant Individual Plan, PAB I and II supervision and oversight, administrative duties, such as faxing, emailing reports, graphing, and communicating with State Case Management. A Service Supervisor is required to develop Individual Plans (IP) for Participants, based on their Individual Service Plan (ISP) from the STATE, incorporating  verbal and written information received from the Participant, the DOH-DD CM, other agencies, the Participants family, or the Participant’s legal or designated representative.  The Service Supervisor must adhere to Medicaid Waiver Standard timelines and develop activities, methods, or approaches with timeframes for achievement to be implemented, including behavioral supports, to achieve the desired goals and outcomes identified in the ISP and or/ Waiver Action Plan.  Service Supervisor gives on site supervision to PAB I and II, once a month or as needed, as well as teaches these workers how to implement the plan.  Duties include data collection, data analysis, monthly reports, quarterly reports, good organizational skills, good communication skills, initiative and follow through

 

Minimum Education

Requires a Bachelors Degree in Social Science or Education and experience working with individuals with developmental disabilities
